Early Warning System for Providers Could Reduce Patient Re-Admissions for Heart Failure
Heart failure is the most frequent condition associated with re-hospitalization. As the provisions of healthcare reform are implemented, providers will soon face reduced Medicare reimbursement if their patient readmission rates for heart failure exceed the national average.
