MHRA Issues Alert for Smiths Medical Fluid Administration Sets

The UK’s Medicines and Healthcare products Regulatory Agency (MHRA) has issued a safety alert for Level 1 normothermic IV fluid administration sets manufactured by Smiths Medical following reports that the device’s heat exchanger assembly may be too long. The oversized sets may not fit with accompanying fluid warmers, making the set unusable. One patient death has been associated with a delay in treatment during an emergency transfusion.
