Hospira Has Second Opioid Lot Recall in a Month on Overfills

Hospira is recalling one lot of hydromorphone hydrochloride injection after two reports of overfills in the syringe-based drug delivery system. The Schedule II opioid agonist comes in a prefilled glass cartridge for use with the 2.5-mL carpuject device. But the affected product reportedly contains more than the 1-mL labeled fill volume. Hospira is investigating the root cause of the recall, which is its second recent opioid recall due to an overfill issue. An April recall affected one lot of its morphine sulfate injection.
