The Centers for Medicare & Medicaid Services (CMS) has determined there was enough evidence to conclude that cochlear implantation is reasonable and necessary for treatment of bilateral pre- or post-linguistic, sensorineural, moderate-to-profound hearing loss in individuals with hearing test scores of less than 40 percent and greater than 60 percent.
